I am happy to provide you with a superbill for services rendered if needed, but do not file claims on behalf. *** Medicaid clients may not pay out of pocket for services, and have to utilize a Medicaid provider due to Medicaid guidelines. NOTE: When considering the use of your insurance for the reimbursement of services you need to be aware that at a minimum I must provide them a diagnosis, progress and plan of treatment. Further, if you have an insurance policy which is reviewed by one of the managed health care companies I must provide them with detailed information as to your personal history, sexuality, HIV status, drug and alcohol use, problems and progress etc. Many times, if your health insurance is managed by an HMO or PPO, the company may only authorize a small number of sessions averaging five or less. NOTICE: You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. We will meet each 1st, 3rd and 4th Wednesday for Individual and each 2nd Wednesday for Group Supervision during the first year and individually only during the second year.
